A1. Language preferred for correspondence
The Ontario Trillium Foundation will correspond with you in either English or French depending on the language you select.
A2. Type of application (please select one):
Select single organization if you are applying on your own and not in partnership with any other organization(s). Select collaborative if two or more organizations are jointly submitting the application.
Select unincorporated branch, chapter or member agency of a registered charity or incorporated not-for-profit organization if you are applying under the authority of your national or provincial incorporated parent organization.
A3. If you are applying as a collaborative, please provide the following information:
Name of Collaborative: Please provide the name of your collaborative, if it has a name. Otherwise, write not applicable.
Number of years working together: Indicate how many years the collaborative has been in existence. If the collaborative is new, write 0.
Number of groups in the collaborative: Please indicate the number of organizations that are formal members of the collaborative and that have signed the collaborative agreement.
A4. Address and contact information:
It is very important to identify a contact person who will be available to clarify the information provided in your application or to provide additional information. Provide the name of the person who can best answer questions about your organization and application. In addition to a telephone number, include an e-mail address and fax number, if available.
A5. What is the eligibility status of your organization? (Please select one):
Select the category that applies to your organization. If you are applying as a collaborative, indicate the status of the lead applicant. If your organization is both a registered charity and a not-for-profit corporation, select registered charity.
A6. If you are applying as a registered charity or a not-for-profit corporation, please provide the year of incorporation or charitable registration and incorporation or charitable registration number.
This information will be found on your incorporation or charitable registration documents.
A7. What is the main sector your organization serves? (Please select one):
Many organizations run programs or deliver services that could be considered to be part of more than one of the four eligible OTF sectors: arts and culture, environment, social and human services and sports and recreation. This question asks you to select the sector that is MOST applicable to the work your organization does.
A8. How many active volunteers and staff are involved in operating your organization?
Tell us how many active volunteers support the ongoing work of your organization. Full-time staff are employees that work at least 30 hours per week. Part-time staff are those that work fewer than 30 hours per week.
A9. What program are you applying for? (Please select one):
Select Community Program if your proposal will have primarily a local or regional impact. Select Province-Wide Program if your proposal will have an impact on a significant portion of the province.
A10. Type of funding:
This question asks you to identify the type of funding you are applying for. In the first section of this question, select the primary type of funding for which you are applying. In the second section of the question, indicate if you are also applying for other types of funding.
Operating funding - for an organization’s ongoing program and/or administrative costs.
Project funding - for one-time activities or activities with a defined time frame.
Capital funding - for renovations and equipment purchases.
A11. Total amount of OTF funding requested:

Enter the total amount of OTF funding requested in dollars and the length of your proposal in months. Then, in 50 words or less tell us what you plan to do with the funding if your grant request is approved.
Sample response for a sports club applying for equipment and marketing to expand number of children participating in the sport:
$50,000 over 24 months to increase the number of elementary school children participating in our sport by providing loaner equipment for learn to play clinics, visiting schools to demonstrate the sport, increasing fundraising activities to offer reduced fees for house league and first-time players and increasing marketing activities.
Sample response for an arts council applying for equipment to improve the quality of its newsletter:
$25,000 over 12 months to increase community awareness and participation in the arts by increasing the quality of our newsletter for a more professional appearance, by upgrading our computer hardware and software, training our staff and volunteers on design techniques and by having our newsletter professionally printed.
A12. Which, if any, of OTF’s four granting priorities will your proposal primarily address? (Please select one):
In the first part of this question, please select the OTF granting priority that your project best addresses. Although your proposal may address more than one of our four granting priorities, your response to this part of the question should indicate the granting priority that is the best fit with your proposal. If your proposal does not address any of OTF’s four granting priorities, select “other”. An application that does not address any of OTF’s four granting priorities may nevertheless be considered for funding provided it complies with our granting principles and program assessment criteria and responds to local or provincial needs.
In the second part of this question, identify which other OTF granting priorities, if any, your proposal will address.
A13. What is the main sector that will benefit from the grant? (Please select one):
Some proposals have impact in more than one of the four eligible OTF sectors: arts and culture, environment, social and human services and sports and recreation. In the first section of this question, select the sector that most closely applies to your project. In the second section of the question, indicate if your project will have an impact on other sectors.
A14. Some proposals are directed at residents of a community in general while others are directed at a particular group or groups.
If your proposal is directed at residents of a community in general, answer yes.
If you proposal is directed at specific populations and/or age groups, answer no and indicate which specific populations and age groups are served by your proposal.

Indicate the population of the community you serve. If your organization serves more than one community select all the categories that apply.
A16. How did you hear about OTF? (Select all that apply):
By telling us how you became aware of OTF, you will assist us to plan more effective outreach and marketing efforts.

Any previous history with the OTF is considered during the review of a new application.
